School Application

  • Income Verification (1040, SNAP/ KTAP/SSI      award letter, etc) 

  • Foster Care Paperwork (if applicable)

  • Proof of Child’s Birth Date 

  • Proof of Residence 

  • Job, K-TAP, school/training verification (full-day/full-year children only)
